These forms are provided for ease of printing and to fill in by hand. There are also printed forms available for pick up in the church reception area.  Forms can be submitted to the church office by either scan and e-mail to admin@gibcjupiter.org, dropping them off, or by mailing to the church office:

Family Information Record

Are you new to the church or need to update your information for our database?   Please submit the completed Family Information Record.

Membership Application

If would you like to apply for church membership, please print, complete and submit the completed Membership Application.  

Ministry Application

Have you been attending Grace Immanuel for more than six months have become a member of the church and would like to volunteer with our Children's Ministry?  You must have an approved Membership Application on file. 

Youth Ministry Application

Those 15 and under who would like to assist in volunteering with the children please use the youth ministry appplication.

For each youth and children ministry event as required by ministry coordinators.
